Registration Fees:
- $285 June 2 through July 31, 2008
- $325 August 1, 2008 and after
CHEER
Registration Fees:
- $215 June 2, 2008 through July 31,2008
- $250 August 1, 2008 and after

MANDATORY PHYSICAL EXAMS

All participants must have a COMPLETED MEDICAL FORM.
Santa Cruz Pop Warner will be hosting
PHYSICAL EXAMS on July 9th
next WEDNESDAY at 5:15 PM (until the last one is done)

Exams will be at the Santa Cruz Medical Rehabilitation Clinic
1529 Seabright Avenue - Santa Cruz
The cost is $20, and all proceeds are returned to
Santa Cruz Pop Warner as our annual fundraiser.

CHEER UNIFORM Fittings will be held at the same time.

for kids ages 11 thru 15

at 5:30 PM

Please bring any missing documents to ANY these events noted above.

Required Documents: Birth Certificate, Complete Report Card, Player Medical Form (signed by doctor from a physical exam), wallet-size Photo, and all Fees Paid in Full.
We must have ALL your REQUIRED DOCUMENTS before any participant
can begin practice on Friday, August 1st.
