The 2008 SEASON is HERE!

Daily Practices are

at Harvey West Park

Santa Cruz

All Teams - Football and Cheer are practicing Monday thru Friday in the month of August. Practices are generally 5:30PM to 7:30PM unless dictated otherwise from the Head Coach.

Before any participant can BEGIN practice we must receive the following:

• Birth Certificate or copy of Passport page
• SIGNED National Pop Warner Player Contract (given at this meeting)
• Full year School Report Card for school year 2007 / 2008
• Signed and completed National Pop Warner Player Physical Form
• Head shot Photo (minimum size 2" x 3") - please print child's name on back of photo
• And All Fees paid in full

All participants must have a COMPLETED MEDICAL FORM.
